The Paediatric Nurse Led Clinic provides local support for community clients in accessing telehealth appointments with a Paediatrician. Paediatrician consults generally require a GP referral.
The Paediatric Nurse can assist with:
- Gathering of relevant information to support referral
- Liaison with GP and Pediatric services
- Attending Paediatric telehealth appointment with client
- Support with follow-up including support with service navigation and treatment education
Inclusive Criteria:
A referral is required, and this can be made either by:
- the SHDH Allied Health Paediatric Lead
- a General Practitioner
- Paediatricians – who have local clients who require follow up via telehealth
Eligibility limited to:
- Children aged 2 months to 17 years residing in the Swan Hill district
- Parent or legal guardian must attend appointment with child
- Reason for referral must be with scope of practice of the Paediatric Nurse
Reason for referral may include:
- Developmental concerns including delay, challenging behaviours, continence issues or learning difficulties
- Growth concerns including failure to thrive, feeding difficulties or obesity
- General health issues including asthma, ADHD, school or adolescent issues
Exclusion Criteria:
- Children who require complex health treatment that is deemed by their physician as unsuitable for management in an out patients community health setting or require physical examination/treatment that cannot be undertaken via telehealth
- Premature and young infants- up to 2 months of age
- Children with Type 1 or Type 2 diabetes as these children can be referred to SHDH Diabetes Education Service
- Children with acute illness requiring immediate care
- Children requiring administration of medication by a health professional
- Self-referral to service is not available
Triage: The Paediatric Nurse will triage referrals according to priority
